In situations where the client has had no contact with the treatment provider for three months, nor is there a plan in place for further contact, the date of last service contact should be used. Refer to the glossary item Cessation of treatment episode for alcohol and other drugs to determine when a treatment episode ceases.

The date must be later than or the same as the treatment commencement date for the episode of treatment for alcohol and other drugs.
