For the purposes of activity based funding reporting, this item can be derived from existing data elements for admitted and residential patients.

Admitted patients: this item can be derived from Episode of admitted patient care—separation date, DDMMYYYY

Residential patients: this item can be derived from Episode of residential care—episode end date, DDMMYYYY

For the purposes of activity based funding reporting, the following information is applicable for ambulatory patients:

Statistical discharge (end of reference period) - this item must contain the last date of the reference period, which may not correspond with a reportable service contact.

Formal discharge - this item must occur on the same date as a reportable service contact.

Formal discharge may occur due to a discharge from the service or a period of inactivity. If the episode of mental health care has a period of inactivity that is greater than 120 days, the episode of care should be ended. In the case of an episode of care being ended for the reason of inactivity, the episode end date will be the date of the last service contact in the episode of care.
