The Functional Independence Measure (FIM™) is an instrument which indicates a patient's disability level.

FIM™ is comprised of 18 items, grouped into 2 subscales - motor and cognition.

The motor subscale includes:

  • Eating
  • Grooming
  • Bathing
  • Dressing, upper body
  • Dressing, lower body
  • Toileting
  • Bladder management
  • Bowel management
  • Transfers - bed/chair/wheelchair
  • Transfers - toilet
  • Transfers - bath/shower
  • Walk/wheelchair
  • Stairs

The cognition subscale includes:

  • Comprehension
  • Expression
  • Social interaction
  • Problem solving
  • Memory

Each item is scored on a 7 point ordinal scale, ranging from a score of 1 to a score of 7. The higher the score, the more independent the patient is in performing the task associated with that item.

CODE 9 Not stated/inadequately described
This code should be used in circumstances where the FIM score code has not been collected. This code is not to be used on FIM score data collection forms.

This data element is used to report a score for one of the 18 items that comprise the Functional Independence Measure instrument. It should be reported in an array of 18 to gain a full (FIMTM) score. The total (FIMTM ) score ranges from 18 to 126. If a response to a question within the score contains a value of 9 (not stated/inadequately described), this should not be included in the calculation of the total (FIMTM) score.
