Definition:![]() | Continuity of carer means care provided, or led, over the full length of the episode of care by the same named carer. Relational continuity is provided by the same named caregiver being involved throughout the period of care even when other caregivers are required. Other caregivers may be involved in the provision of care, either as a backup to the named carer or to collaborate in the provision of care, however the named carer remains to coordinate and provide ongoing care throughout. |
