Indicate whether the person with cancer has experienced any late effects related to cancer and/or cancer treatment.

This does not include issues relating to the recurrence of cancer or progression of the disease. Late effects are as a result of the treatment, the disease process, or both. Examples of late effects relating to cancer are infertility, hyperthyroidism, scoliosis, cognitive impairment and congestive heart failure.
