This code set outlines support services that may be used by a patient. CODE 1 Educational support Information about the disease, treatment and outcomes given to the patient. This may be through physical or electronic methods. CODE 2 Community service Support services provided by volunteers or community groups without specific professional training. CODE 3 Psychiatrist Assessment, treatment and/or consultation with a trained psychiatrist or psychiatry registrar. CODE 4 Psychologist Assessment, treatment and/or consultation with a registered psychologist or provisional psychologist. CODE 5 Specialist nurse/nurse counsellor Assessment, treatment and/or consultation with a nurse with specific training in counselling or specialist training in the disease/disorder area. CODE 6 Professional or volunteer support group Involvement in a professional or volunteer support group that relates to the disease/disorder or the associated effect of the disease/disorder. CODE 7 Individual peer support Contact with another individual who has had or currently has the same or similar disease/disorder, expressly for the purpose of being with provided with support in regards to their disease/disorder. This may be in person or over the telephone. CODE 8 Counsellor/bereavement counsellor The patient has been treated by a counsellor or bereavement counsellor in regards to their current disease/disorder. CODE 9 Pastoral care Care given by chaplains, clergy or spiritual advisors from any religion. |
