Service requests include both referral (e.g. a written referral from a general practitioner), and other informal requests for service (e.g. self-referral for an unplanned 'walk-in' service).

The service request should not be from another provider within the same outpatient clinic but from an organisation external to the outpatient clinic.

A client/patient may be referred (by Unit A) for services to an organisation that has multiple registered health service units. That client/patient may then be referred from one health service unit (Unit B) within the organisation to another (Unit C). In these circumstances the 'source of referral' for Unit C is Unit B (not Unit A).  For example if a patient is referred by his/her General Practitioner (GP) to a general medical clinic, such as an aged care clinic, then after assessment at this clinic the physician refers the patient to an allied health clinic, the service request source for the allied health outpatient clinic is 'Other outpatient clinic', whereas the service request source for the general medical clinic is 'General Practice'.
