Definition: | The period of time between the start and cessation of community based palliative care, during which a patient with a life-limiting illness and/or their carer(s)/family/friends receives services in a community-based setting from a specific agency to improve their quality of life. |
Collection and usage attributes | |
Guide for use: | An episode of community based palliative care commences when the patient has been assessed and accepted for care by the service provider and the patient has consented to receive care. The episode ends when the patient dies, or discharge (case closure) occurs for another reason. During an episode of community based palliative care, a patient may be admitted to hospital (or a hospice) to receive care. The community based episode of care would continue as long as there is an expectation that the patient would be returning to the care of the community based palliative care service provider. |
