Guide for use:

A case management plan is a personal plan or a support agreement that usually has a statement of the person’s problems or needs, some goals for the person and strategies to achieve those goals. It is usually developed between the person and agency as a result of an assessment process.

Collection methods:

The permissible values for this data element are used to form the response categories to the question:

"To what extent does the case worker think the client has achieved their goals over the past reporting period?"

Comments:

This item is answered from the perspective of the agency worker. It is answered at the end of a reporting period as to whether the case management goals were achieved.
