Description:Help on this termPatients with one, two, or three or more chronic conditions is measured as the number of people who visit the GP at least once in a year who had one, two, or three or more chronic conditions per 100 people who visit the GP at least once in a year. Results are disaggregated by Medicare Local catchment peer groups.

National Health Performance Authority analysis of BEACH SAND data (2005-2013) weighted to the attending patient population using Department of Human Services, Medicare Benefits statistics, 12 months ending 30 June 2013.

Number of GP consultations with patients who had one, two, or three or more chronic health conditions during the data period. The 18 common chronic conditions selected in this substudy are:

  • Asthma
  • Anxiety
  • Cancer (malignant neoplasm)
  • Cerebrovascular disease/Cerebrovascluar accident (CVD/CVA)
  • Chronic back pain
  • Congestive heart failure 
  • Chronic obstructive pulmonary disease (COPD)
  • Depression
  • Gastro oesophageal reflux disorder (GORD)
  • Hyperlipidaemia
  • Hypertension
  • Ischaemic heart disease
  • Osteoarthritis (OA)
  • Peripheral vascular disease
  • Rheumatoid arthritis (RA)
  • Sleep disorder
  • Type 1 Diabetes
  • Type 2 Diabetes
