A pinhole perforation is present in the tympanic membrane.

CODE 2   Medium

A perforation is present in the tympanic membrane, that is larger than a pinhole but smaller than a subtotal perforation.

A subtotal perforation is present in the tympanic membrane: the pars tensa is absent but the perforation does not include the annulus.
 
CODE 4   Total

A total perforation is present in the tympanic membrane; the tympanic membrane is essentially absent.
