Lead V4R represents a lead placed on the chest aligned with the right mid-clavicular line, in the 5th intercostal space. The measurements from this lead can identify right ventricular infarction.

Lead V4R should be performed in the context of inferior infarction, especially in the presence of haemodynamic compromise.
