Guide for use: | CODE A1 General adult version This code is used for the Health of the Nation Outcome Scale (HoNOS) clinician-rated measure general adult version. It applies when rating consumers aged 18-64. It should be used in conjunction with Person—level of psychiatric symptom severity, Health of the Nation Outcome Scale score code N. CODE G1 HoNOS 65+ version This code may be used for the HoNOS clinician-rated measure older person version. It applies when rating consumers aged 65 and over. Please note, this version is not currently recommended for use in Australia. It should be used in conjunction with Person—level of psychiatric symptom severity, Health of the Nation Outcome Scale score code N. CODE G2 HoNOS 65+ version 3 This code may be used for the HoNOS clinician-rated measure older person version. Please note, this version is not currently recommended for use in Australia. It applies when rating consumers aged 65 and over. It should be used in conjunction with Person—level of psychiatric symptom severity, Health of the Nation Outcome Scale score code N . CODE 01 Version 01 This code is used for the following surveys:
CODE M1 Last 4 weeks version This code should be used for the version of the K10 that rates the last 4 weeks, referred to as K10+ because it includes all of the questions in the K10 as well as some supplementary questions. Use this code in conjunction with the Kessler Psychological Distress Scale last 4 weeks cluster. CODE 31 Last 3 days version This code should be used for the Last 3 days version of the K10. The Last 3 days version is a variation of the K10 designed for use in inpatient settings where the episode is of less than 3 days duration. It is used in conjunction with Person—self-assessed mental health status, symptom frequency scale code N. CODE PC101 Parent Report Measure for children aged 4-10 years, Baseline version, Australian Version 1 This code is used when the Strengths and Difficulties Questionnaire (SDQ) is initially completed by a parent for their child aged 4-10 years. It should be used in conjunction with the Strengths and Difficulties Questionnaire cluster, which further specifies which questions apply to each version of the survey. CODE PC201 Parent Report Measure for children aged 4-10 years, Follow up version, Australian Version 1 This code is used when the SDQ is completed by a parent subsequent to the initial survey, for their child aged 4-10 years. It should be used in conjunction with the Strengths and Difficulties Questionnaire cluster, which further specifies which questions apply to each version of the survey. CODE PY101 Parent Report Measure for children aged 11-17 years, Baseline version, Australian Version 1 This code is used when the SDQ is completed by a parent for their child aged 11-17 years. It should be used in conjunction with the Strengths and Difficulties Questionnaire cluster, which further specifies which questions apply to each version of the survey. CODE PY201 Parent Report Measure for children aged 11-17 years, Follow up version, Australian Version 1 This code is used when the SDQ is completed by a parent subsequent to the initial survey, for their child aged 11-17 years. It should be used in conjunction with the Strengths and Difficulties Questionnaire cluster, which further specifies which questions apply to each version of the survey. CODE YR101 Youth Self-report Measure, 11-17 years, Baseline version, Australian Version 1 This code is used when the SDQ is initially completed by person aged 11-17 years about themselves. It should be used in conjunction with the Strengths and Difficulties Questionnaire cluster, which further specifies which questions apply to each version of the survey. CODE YR201 Youth Self-report Measure, 11-17 years, Follow up version, Australian Version 1 This code is used when the SDQ is completed by person aged 11-17 years about themselves, subsequent to their initial completion. It should be used in conjunction with the Strengths and Difficulties Questionnaire cluster, which further specifies which questions apply to each version of the survey. |
