The commencement date is the date on which an admitted palliative care patient commences a new palliative care phase type. Subsequent phase begin dates are equal to the previous phase end date.
The palliative phase of care start date is to be recorded at the commencement of the episode of admitted patient palliative care and at the commencement of every subsequent palliative care phase thereafter in the same admitted patient episode.
